ABOUT THIS EVENT

The onset of COVID-19 brought about new national guidance for hospital discharge. This has resulted in significant changes in hospital discharge processes, including the introduction of a new national discharge to assess process, with the majority of assessments for care now happening after discharge. This can have a big impact on care providers supporting discharges.


This online course has been designed to make sure that care providers are up to date with The Hospital Discharge Policy and Operating Model, as well as the new National Discharge To Assess Model. This training will also support providers to explore how they can work effectively with discharge teams to deliver safer hospital discharge and post discharge assessment, taking a strengths-based approach.

The training will also help create capacity within domiciliary services by effectively assessing correct level of care and ensuring that individuals are provided with the right level of care (to avoid becoming dependent). As providers are an active part of the D2A process, they can also contribute to the decision making around the right level of care.

METHOD OF TRAINING DELIVERY

This course is 3 hours long. The course has a live element (2 hours) and a pre-recorded element (1 hour), and both elements must be completed for successful course completion and certification.

The live session will be delivered online via the Zoom platform. In order to access the live session, you would need to have the Zoom application downloaded on your device. You can download the Zoom application here. Registrants will be sent the Zoom details for the live session ahead of the course start date.

The pre-recorded element of the course will not be delivered live (instead, course registrants will be asked to watch recorded training sessions, read handouts/resources and complete short quizzes, and will be given a deadline for completing this). The pre-recorded element will be delivered to registrants via our training system.

Registrants will be sent further instructions over email on how to access the training system.


Registrants will be expected to complete the recorded element before they join the live training.

  • Describe the National Discharge Operating Standards and the new ‘Discharge to Assess’ model for hospital discharge.
  • Identify how providers can be an effective partner in hospital discharge.
  • Describe the challenges in delivering the new hospital discharge pathways.
  • Describe the importance of taking a strength-based approach during the period of post discharge assessment.
  • Describe how provider information contributes to assessment about ongoing care and support needs.
  • Identify best practice principles in recording and sharing information to facilitate discharge to assess.
